DNA kits can be supplied to professionals or individuals. *
They can be used to profile and/or store DNA of individuals or pets. This DNA can then be used to:
- Determine parentage/inheritance issues
- Forensic/missing persons issues
- Determine inherited genetic diseases
- Establish geneaology of a family
- Diagnose infectious diseases
- Determine predisposition to inherited metabolic disorders & genetics cancers
- Provide “Peace of Mind” knowing that your family’s medical and genetic history is preserved.
Any funeral home electing to purchase a start up kit will receive the following:
- Letter to funeral directors explaining the value of our service.
- Brochures that can be given to their clients/families.
- “Authorization to Collect”, this will release the funeral home of liability.
- “Sample Collection Instructions”, on how to collect each sample.
- “Paternity Sample Collection Instructions”, these instructions are for living individuals.
- “Client Authorization”, this form must be filled out for any parentage/inheritance issues.
- Sample Press Release.
- “The Ultimate Gift of Life after Death”, this is a flyer that can be displayed in the funeral home or can be given to your clients/families.
- Envelope marked “For DNA Storage Only”, this envelope contains three (3) Sterile Applicators that should be returned in the envelope along with all required information.
- “Collection Kit”, this kit contains all sterile items, two (2) Sterile Polyester Tipped Applicators. One to be used for buccal swab and the other to collect blood, then place the blood on the
FTA card. A foil lined envelope to place the FTA card. The plastic bag containing the items should be used to place the hair with root, and an envelope to place all the collected samples.
- A sample “BOOKLET”, this booklet will contain the profile, explanation of the profile, future use of your DNA, a family tree, a certificate of authenticity, and a support page for any questions
you or your client/family may have.
The cost for the start up kit is $29.95 plus $5.00 for shipping and handling
for a total cost of $34.95.
The cost of Profiling and Storage is $350.00
For Profiling Only is $250.00
For Storage only is $150.00
*It should be noted that the funeral directors may charge for the collection of DNA. The collection is done by a licensed embalmer or
funeral director. This creates an audit trail for DNA Analysis, Inc. as well as the family having the trust and confidence in their funeral director.
